用Polybutester做縫合式雙眼皮眼瞼形成術之經驗

The Experience of Monofilament Polybutester in the Suture Technique Blepharoplasty

對於具有薄單眼皮的年輕東方人，縫合式眼瞼形成術是相當簡易的雙眼皮手術。利用縫線的固定，此種術式可提供瞼板舉肌筋膜區域與眼瞼皮層下區域相當程度的黏連，但往往被提出質疑的是“經由這樣所製造產生的雙眼皮效果可以維持多久？”自從 1990 年起，我們對總數計 123 位的病例（平均年齡 25.3 歲）使用 6-0 Polybutester 縫線材料縫合 3 針行縫合式眼瞼形成術。其中 31 位病例（24 位女性，7 位男性）接受長期達四個月至六年的追蹤調查（平均追蹤期 42 個月）。結果顯示所有追蹤病例之雙眼皮皆繼續存在中，且百分之八十一的病例表示滿意或是非常滿意，性別分析顯示男性滿意度更佳。四位病例有雙眼皮寬度些微縮減之現象，併發症方面，僅一位病例有短期流淚之困擾。相對於其他縫線材線， Polybutester (Novafil)具有更佳之彈性與順從性，長期的病例追蹤研究也顯示它非常適合應用於縫合式雙眼皮眼瞼形成術。
One of the simplest way to create double fold in thin oriental eyelid for a young patient is the suture technique. It provides transfixation between levatotarsal and subdermal region, but the question is "How long can it will last?" From 1990, we had performed suture blepharoplasty using 6-0 polybutester pre-2 needles (Novafil 4412-13) with 3 stitches hooked suture for 123 patients (average age of 25.3). Out of the 123 cases, 31 cases could be followed and enrolled in this study. Of these patients, 24 were females and 7 were males, with the follow-up period ranging from 4 months to 6 years. 25 patients (81%) were satisfied or very satisfied with their result. All male patients accepted highly to this method. By the time of the average 42 months post-operative follow-up, double folds were still present in all patients (100%); only 4 cases had evidence of width reduction in the fold we created. Remarkable complications were not noted except one case which had frequent tearing last for a couple of day after operation. Polybutester (Novafil) had a greater compliance and elasticity than other non-absorbable monofilament suture materials. It clearly proves the effectiveness of the usage of polybutester in the suture technique. Not only the technique is simple it also resulted with high satisfactory rates and rarely had any other complications.
